About Ilizarov technique

The ilizarov technique aims at straightening out the bones or increasing the length. It does so by surrounding the affected area in a cylindrically shaped structure made up of rods, rings, and wires, from the outside. 

 

This structure runs through the limbs and thereby attaches the limb with the structure. As a result of this fixation, the bones in the limbs are carefully pulled away from one another. This further breaks up the diseased bone and provides enough space for new bones to grow and take its place.

 

Ilizarov technique treatments

The ilizarov technique surgery addresses bone conditions such as osteomyelitis, pseudo-arthrosis, or fibrous dysplasia. It is also used to make corrections in the limb’s length or treat an infected bone. Because its uses are various, some of them are included here as follows:-

1.Congenital deformities in the limb

Congenital deformities refer to the deformities that a person develops in its foetal period. Sometimes, the incomplete development of bones such as in the thighs or the incorrect development of bones where the child’s bone growth is misaligned, warrants for an ilizarov technique surgery. Pseudoarthrosis in which, during birth, the bones of the child have not completely fused together, is yet another reason to get this surgery.

2. Achondroplasia

In achondroplasia, the person’s bones in the arms and legs are deformed. In other words, it does not grow as it was supposed to. As a result of this, the person’s ability to move around is very limited.

3. Skeletal dysplasia

This condition encapsulates a wide array of bone deformities, including the aforementioned achondroplasia. There are about four hundred types of deformities that fall under skeletal dysplasia, all of which occur as a result of genetic mutation in the body. 

Most skeletal dysplasia can be diagnosed when the baby is in its foetal period. They are all characterised by an unusual growth in the spine, legs, arms, and skull. There are neurological and functional disturbances as well. When the symptoms are severe, the doctor may suggest surgery. 

4. Osteomyelitis

Osteomyelitis is the medical term for the condition of infected bones. The bone gets this infection via the bloodstream or the bone itself directly when it has been injured. Additionally, people who smoke tobacco, have diabetes or kidney problems, are prone to this.

The ilizarov surgery procedure

The surgery happens in two phases.

 

For the surgery in the first phase, the surgeon makes incisions and breaks up the diseased bone. Following this, the wires of the cylindrical structure are inserted and taken out through the other side. 

 

The wiries are fixed with the help of screws in the rods. The surgeon then fixes the cylindrically shaped structure finally to give complete support.

Regular consultations are mandatory for measuring how much space has been created for new bone growth after  breaking the diseased bone and fixing the circular instrument. This may take a few months during which time it is expected the new tissues and bones will grow from the space provided. 

 

In the second phase, when the bone has matured enough and the surgeon feels that there is no need for further growth, the instrument is removed. It is done so via another surgical procedure under general anesthesia.

 

 It may take anywhere between three-twelve months for complete recovery.

 

After this, a brace will be given to the patient to support the healed part. Wearing the brace and doing regular physiotherapy sessions are mandatory for proper recovery.

 

Post surgery procedure

Following the two surgical phases, it is important to follow the instructions given by the doctor. Some of them will include:-

  2. Keeping the leg at an elevated level.
  3. Wearing proper clothes which would not interfere with the frame.
  4. Keeping the operated sites clean and dry as advised by the surgeon.

  6. Taking precautions to avoid falling under any circumstance.
  7. Sleeping on the back to avoid any added injury

Physical therapy is an integral part of healing in this surgery. Sometimes they begin in the hospital and may continue throughout the entire phase the structure is inserted. Regular follow-up consultation is mandatory to track the progress and realign the instrument accordingly.
